MD Anderson Cancer Center Advanced Prac Registered Nurse (APRN/PA) - Discharge Coordinator, Hospital Medicine in Houston, Texas

The primary purpose of the Advanced Practice Provider (APP), in the Department of Hospital Medicine is to promote and maintain the health of HM patients across the care continuum throughout the institution supporting the inpatient hospitalist programs.

Advance Registered Nurse Discharge Coordinator in the Department of Hospital Medicine, specifically will collaboration with attending physicians and other members of the multidisciplinary healthcare team to facilitate discharge planning tailored to each patient's unique needs, ensuring a thorough and personalized approach to care that maximizes patient outcomes. As the Department of Hospital Medicine is a service that operates 24/7/365, nocturnal shifts will also be supported by the hospitalist advanced practice providers for after-hours and weekend coverage, rotating holidays and COVID coverage.

APRN KEY FUNCTIONS

  • Advanced Practice Registered Nurse in the Department of Hospital Medicine is responsible for assessing and conducting comprehensive patient history, physical examinations, and psychosocial assessments.

  • Evaluate patient outcomes in coordination with attending physicians and develop a treatment plan.

  • Monitors laboratory, radiographic and procedural data for patient disposition. Must visually evaluate data and prepare written information.

  • Communicates report findings and test results to physicians, multidisciplinary team and family members.

Promote collaboration and communication among healthcare team members, other colleagues, and organizations to promote and protect the best interests of patients.

Provides health education to the patient and his/her family (i.e. treatment options, management of side effects, anticipatory teaching, etc.).

Participate and collaborate on clinical research protocols. Interviews patients and reviews the eligibility criteria to identify potential patients for protocol studies.

Deliver education to patients and their families. Conducts in-service training for in-house nursing personnel regarding protocol requirements. Acts as liaison with patients, families, physicians, and other personnel.

Participates in the education of Advanced Practice student nurses, house officers and medical students. Instruct coworkers in allied positions in recording patient information and/or data entry.

APRN DISCHARGE COORDINATOR SPECIFIC FUNCTION

Develop relationship with patient to establish a consistent point of contact for the patient throughout the time of admission, discharge and Post discharge.

  • Assess patient, family and caregiver needs upon initial encounter to admission and periodically throughout the continuum of care, including assessing changes in psychosocial, financial and emotional occurring due to treatment.

ยท Facilitates scheduling, logistics, and future appointments.

  • Responds timely to requests to coordinate discharge services for unplanned discharges and communicates, both verbally and in writing regarding discharge plan.

  • Coordinate appointments for discharge follow up with oncology team with support services within established service standards, including social workers, dieticians, therapists, and others as appropriate throughout the treatment journey.

  • Assist the patient with any matters related to treatment goals, advance directives, palliative care, and end-of-life concerns using an ethical framework.

  • Apply knowledge of payer requirements and insurance processes and their impact on referrals and patient care decisions to establish appropriate referrals, as requested or needed

  • Communicate any concerns and perform patient follow-up about timely coordination of appointments, tests and procedures to the appropriate scheduling or care team members.

  • Perform outreach efforts across MD Anderson to establish and maintain positive working relationships with key inpatient care team - faculty, departments, APPs, nursing, case managers, social services, and others.

EDUCATION: Master's Degree in Nursing or Doctor of Nursing Practice. May be waived for applicants who obtained recognition as an Advanced Practice Registered Nurse from the Texas State Board of Nursing prior to 1/1/2003. Graduate education and related Texas Board of Nursing authorization must be consistent with clinical practice area and patient population. All of the following:

LICENSURE: Written authorization from the Texas Board of Nursing to function as an Advanced Practice Registered Nurse (APRN or ACNP or ACNPC or ACAGNP or AGACNP or ACNPC-AG or AGACNP or AGNP or ACPNP or ANP or AGNP or AGPCNP or FNP or GNP or PNP or PMHNP or WHNP or APNP). Related authorization must be consistent with clinical practice area and patient population. Basic Life Support (BLS). Pediatric Advanced Life Support (PALS) If working in the following departments: Pediatric - Patient Care (113100). MAY BE REQUIRED: Authorization for limited prescriptive authority by the Texas Board of Nursing.
