This position will be 100% onsite.
The MD Anderson Clinical Transformation Center aims to identify, implement, and accelerate innovations, that leverage digital technologies, in the experience and delivery of cancer care. Our team works with a broad range of internal (e.g. ICCI, Innovation Center) and external collaborators, in a mutually reinforcing manner, to design and implement new models of care delivery that are affordable, patient-centered and value driven.
The Program Manager - Digital Health & Remote Patient Monitoring will focus on digital health and work within a multi-disciplined team that translates human insights into tangible products, services, and experiences. The Program Manager will be primarily responsible for ensuring the successful integration and implementation of technology solutions (remote patient monitoring, video conferencing, chatbots, wearables) into a variety of clinical practice settings. These approaches are intended to augment care delivery in between in-person visits with the clinical team. Furthermore, the Program Manager will aid in providing a digital inclusion process that includes facilitating home connectivity, device and digital assistance, and management of remote monitoring for patients. The Program Manager will help to drive operational transformation, strategic direction and organizational readiness for a more digitally enabled care paradigm through their management of assigned projects, programs, strategic initiatives and their governance.
The Program Manager will provide support to two main projects within the Virtual Care portfolio: Care and Monitoring at Home , Remote Patient Monitoring .
The ideal candidate will have a Bachelor's degree and at least five years of program/project management experience. Preferred candidates will have a desire to work with programs involving digital health and technology that merge with organizational strategy.
JOB SPECIFIC REQUIREMENTS
Assist with the ideation and implementation of innovative care models that involve digital technologies. Expectation is to assist with the dissemination and scale strategy of these new care frameworks across various care settings (Acute Cancer Care Center (ACCC), inpatient, ambulatory, community) and service lines (surgery, medical oncology, hospital medicine) as appropriate.
Assist with care transitions in the Care and Monitoring at Home program related to emergent transfers to the ACCC (care escalation) or uneventful discharge (i.e. ensure appropriate follow-up with primary oncology team)
Manage data related to the implementation of remote patient monitoring programs (i.e. census, clinical data, logistical data)
Provide technical support for remote patient monitoring platforms (Wi-Fi connectivity, notification issues, downloading issues, password issues, Bluetooth connectivity issues)
Track appropriate patient-level quality outcomes (specifically quality of life) for program monitoring and evaluation
Manage program completion and offboarding of remote patient monitoring platforms
Place orders for relevant digital technology and manage logistics (sourcing, procurement, and storage)
Maintain appropriate documentation for regulatory compliance, privacy protections, and mandated reporting requirements
Interface with financial services with respect to patient out-of-pocket obligations for the provision of services, as appropriate
Adhere to clinical departmental policies and procedures, which include Joint Commission standards, Patient Safety initiatives, Patient Rights, and Health Insurance Portability and Accountability Act Privacy Standards
Attend relevant stakeholder and advisory meetings to support overall program implementation and evaluation
Attend necessary training and learning opportunities that expand content knowledge and best practice
Other duties as assigned Patient Enrollment and Education
Identify eligible patients for remote monitoring and other digital care use-cases
Enroll eligible patients (i.e. satisfies inclusion and exclusion criteria) into remote monitoring programs
Track patients who are admitted to ACCC and facilitate ad-hoc "off boarding" of patients due to patient/care-giver preference, care transition to hospice or unforeseen death
Obtain patient consent and maintain log of patients enrolled in programs
Maintain communication, via remote monitoring platform, with patients who are at risk for barriers to treatment (transportation needs, housing insecurity). Make confirmatory calls and emails to these patients regarding future appointments.
Facilitate, as appropriate, the scheduled delivery of summary statements outlining vitals and Patient Reported Outcomes (PROs) to the care team and patient
Systematically assess digital health literacy amongst MDACC patients seeking acute care and provide access to available community resources related to digital equity that can enable patients to overcome barriers
Provide patient and caregiver education for the appropriate use of remote monitoring platform (technical components, goals of the program, and patient workflow)
Other duties as assigned.
Required Education: Bachelor's degree.
Preferred Education: Master's degree.
Required Experience: Five years related experience to include three years program management experience. May substitute required education degree with additional years of equivalent experience on a one to one basis. With preferred degree, three years of required experience.
Preferred Experience: Strong background in program/project management strongly preferred.
